4-a side mixed tournament. All teams must have a minimum of one woman playing on court. One nominated person needs to complete this form with their contact details.

Payment will only be accepted via bank transfer - please ensure you use your team name as the reference when paying us so we are able to track payments. Your space will only be secured once payment has been received, which we will then confirm with you via email.

At the time of creating this form, it is not known whether there will be any track and trace requirements, we will update accordingly when more information is received from the government. Until then it will be the responsibility of the person compeleting this form to have the contact information of all team members.
