JAM Advent Service Permission Slip
This year JAM will be again be leading the congregation in Advent Worship on Wednesday, December 18th at 6:30pm. We are looking forward to having all the children in JAM participate!

Please take a few moments to complete this permission slip to let us know if your child will be able to be a part of this special event.

SPECIFIC MEDIA COVERAGE AND CHURCH PUBLICATIONS - In addition to the above situations, there may be times the media (newspaper, television or radio) or staff may take photographs, audio/video children or interview children in a manner that would individually identify a specific child. The church may display pictures and projects in a variety of ways that reasonably portray programs of St. Paul Lutheran. *
