Lyle Miller 4-H Award Application
                                       (Submitted by currently active 4-H Club Leaders only)
The Trustees of the New Salem Congregational church of Lebanon established a trust fund and a memorial for Lyle Miller, a dedicated 4-H leader and supporter. The earnings from the fund will be used to present an award to the most outstanding members each year.

Upon recommendation of the Awards Committee and approved by the Boone County 4-H Council, the following procedure was established to select the recipients of the awards:
1. Awarded at the Annual Boone County 4-H Achievement Awards Program to outstanding members is determined by the awards committee.
2. Members cannot win the award more than once in every 3 years.
3. 4-H’ers must have completed 3 years of project work before they can be considered for the award.
4. Award recipients will be selected from nominations made from local 4-H Club leaders. Leaders should take into consideration the following qualifications for the award:
           a. Attendance at club meetings
           b. Leadership responsibilities fulfilled
           c. Citizenship
           d. Project Achievement
5. One nomination will be accepted from each Boone County 4-H Club. Therefore, clubs with more than one leader should collaborate in completing the nomination..
6. Deadline to submit nominations is June 20, 2023.
