PARENT’S APPROVAL, STUDENT, FAMILY, AND PARTICIPANT WAIVER 

This form is to give permission to your student to participate in PTA sponsored events (before, during and after school hours).

These events include:
All student clubs, Rock-A-Thon, Book Fair, Back To School Party, Family Art Night, Trunk-Or-Treat, Science Night, Game Night, Family Dance, Lake Loves Reading, and any other event added to the calendar that includes all students. 

This form does not in any way require PTA membership, as PTA includes all students at Lake, regardless of membership.
