This form is to receive your 2nd-semester learning choice and make the necessary planning.

Please fill out ONLY if you are planning to make a change to your student's current education model.

• If you have more than one child attending DAS, you must make a separate submission for each of your children

• You will have to use your Student ID and the Username to make your selection (sent via email previously)

• Your student will be placed in the learning model according to your selection in this survey. If you do not fill out this form your child will REMAIN in his or her current education model.

Please have your student USERNAME and STUDENT ID handy before starting your submission.
