All participants must be in employment, including self-employed, employed student or freelancer, (unless recently redundant  or furloughed due to COVID-19) and must fully complete this form to take part in this trade union organised and Scottish Government funded training course.

It is a condition of Scottish Government funding for all participants to complete this form, and to complete the equalities and evaluation forms provided.  If you do not wish to do so you are ineligible to take part.
