FOR VALUABLE CONSIDERATION, receipt of which is hereby acknowledged, I hereby grant the undersigned photographer (“Photographer”) the irrevocable right and permission, throughout the world, in connection with the photographs taken of me, or in which I may be included with others, the following: (a) the right to use and reuse, in any manner at all, said photographs, in whole or in part, modified or altered, either by themselves or in conjunction with other photographs, in any medium or form of distribution, and for any purposes whatsoever, including, without limitation, all promotional and advertising uses, and other trade purposes, as well as using my name in connection therewith, if he so desires; and (b) the right to copyright said photographs in his own name or in any other name that he may select. I waive the right to inspect or approve any use thereof. I hereby forever release and discharge Photographer from any and all claims, actions and demands arising out of or in connection with the use of said photographs, including, without limitation, any and all claims for invasion of privacy and libel. This release shall inure to the benefit of the assigns, licensees and legal representatives of Photographer, as well as the party(ies) for whom he took said photographs. *

• I agree to attend the time slot for which I am scheduled*. I understand that it is extremely important that every performer be ready when called. For the good of the show and out of respect for the other performers and staff, the director must know my time slot is covered. *Parents may discuss conflicts via email, kelly@secondactsa.com.


• I understand that arriving more than thirty minutes late may affect my time slot. 


• I understand that I am committing to performing during my assigned time slot. All performers should arrive between 6:30-6:45 to check in and be in the house a minimum of 30 minutes before their slot. Should you need a specific sound check or set up requirements, please reach out to Kelly Lepeska to arrange. All performers must be at least 11. years of age and all content should be considered "family friendly." House opens to audience at 6:50. There is no admission to attend, however a small donation is requested at the door.

Second Act Productions, LLC does its utmost to ensure the health and safety of its participants. Parents of minor children participating in SA productions are advised to be mindful of the fact that performance in (and rehearsing for) live theater has the potential for risk of danger (including, but not limited to, use of costumes, use of stage props, use of stage weapons such as blades or firearms, use of stage pyrotechnics, etc). 


I acknowledge that my participation in this event is voluntary. I acknowledge that I have been advised of the potential dangers of performance in live theater. After deliberation, I elect to voluntarily participate in this production. I agree to exercise due care and diligence while participating in this production, and to heed all instructions given to me with regard to safety and procedure in the rehearsal hall / performance venue and surrounding area. I agree to hold harmless (and waive any claim or action against) Second Act Productions, LLC, its agents, owners, directors, board of directors, producers and staff from any liability, fault or action arising from participation in this production.

There is no performance fee, however there will be a suggested donation for audience members at the door.
