Defenders Basketball Registration Form 2023-2024 Season

COMPLETE THIS FORM ONCE FOR EACH ATHLETE YOU REGISTER.
This registration form has seven sections. Below is a description of each section. You must complete this form once for each player. Do not try to list multiple players. If you must register more than one athlete, you may want to make note of some of the information you enter, such as your Crew Preferences which you can make the same for each athlete you register. Please click the "Next" button at the bottom to begin filling out the form. When you are finished, there will be a link available to complete the form again if you have more than one athlete to register.

1. Parent Information
You must list at least one parent or guardian's contact information. If you only list one, then you must list an additional emergency contact under that section.

Defenders Basketball is a completely volunteer/family run organization. All families are expected to serve on crews to help make the season possible. Each family is expected to serve on one crew for every player from your family involved in the program. For example, if you have 2 players then you will need to serve on 2 crews, etc. You can have other family members and athletes themselves serve on some crews. Please review the PDF with crew descriptions included in the same email you received with the link to this form (or click here: Crew Descriptions). You will be asked to give your crew preferences in section 3 of this form.
4. Waiver of Liability
Contains the Waiver of Liability form that must be filled out for each player and turned in at the Parent Information Meeting or by the first practice. Players will NOT be permitted to practice without this form signed and turned in.
5. Emergency Health Care Authorization and Contacts
If you only listed one parent or guardian earlier in the form, then you must list at least one additional emergency contact in this section.
6. Parent and Player Commitment Forms
One commitment form for EVERY player must be filled out (initialed/signed) by both the player and his/her parents and turned in preferably at the Parent Information Meeting, but no later than the first practice.
