I/we desire to have my/our child(ren) participate in athletics during the 2020-2021 school year. In consideration for my/our child(ren)’s participation in athletics during the 2020-2021 school year, I/we hereby agree to the terms and conditions set forth herein.  By signing this agreement, I/we acknowledge the contagious nature of COVID-19 and the inherent risks of exposure at Garrison Forest School to those who may be infected with COVID-19.  I/we acknowledge that Garrison Forest School has implemented reasonable preventative protocols, policies and procedures designed to reduce the spread of COVID-19 on its campus for the 2020-2021 school year.  I/we voluntarily assume the risk that my/our child(ren) may be exposed to or infected by COVID-19 by participating in Garrison Forest School’s athletic program.  I/we acknowledge that my/our decision to have my/our child(ren) participate in Garrison Forest School athletic program is voluntary, and that my/our child(ren) may choose at any time to cease participating.

I/we acknowledge that my/our child(ren) and I/we are expected and required to abide by Garrison Forest School’s policies and procedures to limit the spread of COVID-19, as outlined in its COVID-19 Protocols and as may be changed from time-to-time.

By signing this Acknowledgement of Policy and Waiver of Liability, with full appreciation of the risk involved, on my/our own behalf and on behalf of my/our child(ren), I/we hereby voluntarily release and forever discharge Garrison Forest School, its trustees, officers, employees, agents, and contractors from any and all legal or financial responsibility for any personal injury, disability, illness, damage, medical expense or death, arising from or related to exposure to COVID-19, whether such exposure occurs before, during or after my/our child(ren)’s return to Garrison Forest School’s campus.  I/we agree, for myself/ourselves and my/our child(ren), not to make any type of legal or equitable claim on Garrison Forest School, or any of its trustees, officers, employees, agents or contractors with respect to any exposure I/we or my/our child(ren) may have to COVID-19, whether or not it arises through the negligence, omission, default or other action of anyone affiliated with Garrison Forest School, including fellow students. I/we further agree that if any such claim is made, I/we will indemnify and defend Garrison Forest School with respect to any such claim.
