GENERAL CLINIC INFORMATION:
Please read the following carefully before you continue:
- Each appointment time is 1 hour in length for 120$ (tax included). This includes GST and is subject to change at any time.
- Clinic hours are 9am - 4pm, with last appointment beginning at 3pm. I do not treat evenings or weekends. These hours can vary by day or week and are subject to change at any time.
- Payments methods accepted are: cash, Visa, Mastercard or e-transfer (no debit or amex). I will provide you with a receipt via email.
- Due to administrative constraints, I am unable to provide direct billing services or communication on your behalf to insurance companies.
- If you have insurance coverage, it's up to you to contact your provider to confirm that Athletic Therapy is covered.
- Due to administrative constraints, I am unable to treat people with an open injury case (ex. ICBC, motor vehicle injuries, WorkSafeBC claims, etc). Feel free to contact me once your case is closed.
- This is an application to be put on the waitlist, there is no guarantee that you will be receiving an appointment time.
- Your information will be kept confidential and used as a screening process only to ensure we are a good fit to potentially enter into a client/Athletic Therapist contract/treatment agreement.
