SGCS 2021-2022 Student Enrollment Form
Welcome to the SGCS 21-22 Enrollment Form.  Please complete each of the 12 sections below.  Your form will not be considered complete until you hit the submit button at the end of section 12.  In addition, you will need to email the documentation below to enroll@sevengen.org to complete your application.  A confirmation email will be sent to you once a completed application is received by the school.  

* Copy of the Students Birth Certificate
* Proof of Address (Ex: Utility bill, bank statement, lease agreement, or drivers license)
* Updated Immunization Records
* Most Recent IEP (Only if applicable)

Once you begin you must complete the form in its entirety.  The average time to complete this application is 15- 30 minutes. Applications will begin to be accepted on Enrollment Night, January 20th, 2021.  On March 3rd, 2021 the enrollment lottery will take place.  All completed applications will be entered into the lottery.  An acceptance letter will be emailed out to all accepted students on March 5th, 2021.  If you have any questions please do not hesitate to email enroll@sevengen.org.  

Please Note: A child enrolled in another public school or a nonpublic school or private school cannot, at the same time, enroll in a charter school.
