Pocasset Dental Screening Opt Out
Rules and Regulations for School Health Programs in the State of Rhode Island mandate every student through fifth grade shall be given an annual dental examination. In Tiverton Public Schools, dental screenings are performed annually on all children in grades K through five, and in grade nine.  The school department is notifying you of your child’s upcoming dental screening scheduled on Thursday, February 29th, with Cora Rocha, Licensed Public Health Dental Hygienist
We realize that many children see their own family dentist regularly and do not require such a screening.  If your child has had a dental exam within the last six months, or if you wish to decline the school dental screening and have your child examined by your own dentist, please complete the section below prior to Friday, February 16th.

*Please complete individual form for each Pocasset student you wish opt out of screening.
