Welcome to St. Anne CYM! This form is to be completed and submitted by the PARENT of each participant.

Completion of this form is required by each participant. Only one submission required per school year unless information changes.

An additional PRINTED and signed "LIABILITY, MEDICAL AND CONSENT FORM" is required by each participant prior to attendance at their first in-person event. That form can be downloaded at www.stannerh.org/cym or picked up in the parish office M-Th 9:00am-4:00pm

I  (parent/guardian  above),  grant  permission  for  my child,  (youth  named  above),  to  participate  in  the St.  Anne  Youth  Ministry.    CYM events  will  take  place  under  the  guidance  and  direction  of  parish employees  /  or  volunteers  from  the  parish  and/or  Diocese  of  Savannah,  Georgia.    I  agree  to  support and  participate  in  all  communications  relating  to  the  weekly  events. I  understand  that  events  will  be posted  to  the  website at www.stannerh.org/cym and in the weekly bulletin and grant permission for my child to attend. *
 I  hereby  grant  permission  for  photographs/videos  to  be  taken  at  the  St.  Anne  Youth Ministry  functions  of  my  child  and  used  in  communication  mediums  such  as  local  newspaper,  Diocese Publications,  Parish  Bulletin,  Website,  Instagram,  St.  Anne  Catholic  Youth  Ministry  Web  Page,  and St.  Anne  Facebook  Group  and/or  Page. *
I give permission for my child to participate in CYM events hosted via electronic means, such as Zoom. *
I give permission for my child to ride to CYM events with adult volunteers who have been cleared by the Diocese of Savannah *
I give permission for my child to ride to and from CYM events with teen drivers who I approve. I understand that myself, the teen driver, and their parents are responsible for my child the entire time he/she is in their car. My child will only be guaranteed supervision when on CYM property during specified event hours. *
I understand that I must complete and sign a printed copy of the LIABILITY, MEDICAL AND CONSENT FORM available on the website before my child can participate in any in-person events.
My child has read and agrees to the guidelines set forth for CYM in the Code of Conduct (available online or contact the youth minister)
