The diocese has given all diocesan youth ministries, including Holy Trinity Youth Ministries (HTYM), a new participation form that must be completed, by both adult leaders and teen participants. It's pretty straight-forward, and you'll learn more as you progress through this quick form, here.

All HTYM volunteers and participants of in-person activities are to fill out the below online waiver or the printed version. This form is two pages (sign after the next page). If you'd prefer a hard copy/paper version of this waiver form -- to print, complete, and bring to the parish office in person -- or a Spanish version, please email Paul.
