Junior Colonial Misconduct Reporting Form

Junior Colonial Field Hockey understands that it can be difficult to report suspicions or allegations of misconduct towards athletes (especially child abuse or neglect, including child sexual abuse), and respects the sensitive nature of all reports. When filing a report you are encouraged to provide as much information as possible as it helps to effectively respond to the reported misconduct.

You may report anonymously by omitting any identifying information. However, please be aware that this may limit Junior Colonial's ability to respond to the information you provide. If you wish to report anonymously, either leave identifying information fields blank or put "Anonymous". 

  • In New Jersey, any person having reasonable cause to believe that a child has been subjected to abuse or acts of abuse should immediately report this information to the State Central Registry (SCR). If the child is in immediate danger, call 911 as well as 1-877 NJ ABUSE (1-877-652-2873).
