Please fully complete this form if you would like bus transportation this fall and did not indicate that in the survey this Summer.

**Under Plan B students will be expected to wear a mask at all times on the bus, and the number of students on each bus will be reduced to support social distancing.

**Students who attend Laney but do not live in the Laney district (ex: Open Choice, etc.) are not eligible for bus transportation.

**All students are expected to uphold and follow all county and school rules at designated bus stops and while on the school bus. Violations of these rules could lead to a loss of bus privileges.

**Survey must be completed no later than Thursday, September 24, 2020.
