Dear Marguerita Family,

Thank you for participating in the the 2019 Marguerita PTA Jog-A-Thon.  This year we will be having two grand prize categories - individual and family.  If you would like to combine your family's pledges for top fundraising Family Grand Prize consideration, please provide us with the names, grades, and teachers of all your children who attend Marguerita School. We will use this list to combine totals pledges collected. We reserve the right to verify family relationships.

The collection amount of the individual student will be evaluated for both the individual prize and combined with siblings for the family grand prize to give the student the best chance of winning a grand prize in either category.

However, to ensure more students have a chance at the grand prize, an individual cannot be the winner of both grand prizes. In case your child/family is the top fundraiser in both categories, please select the prize you wish to receive and complete this form by Nov 15th.
