Please fill out this form to participate in the IA/Rising Risk Professionals Trivia Night on Thursday, December 8th at Wrigleyville North Bar (3900 N Sheridan Rd). The event will begin at 6p and trivia to start at 6:30p. To participate, one member of your team must complete this form. Teams can be formed in either full groups of six or in teams of three, where you will be paired with another group of three to make a full team.
Once the form has been completed, you will receive a confirmation email with your team number. Due to the limited capacity at the bar, you MUST receive a confirmation email to ensure your team has been signed up for the event.
