5. Environmental factors

Participation in this study is by only by invitation and purely voluntary.

If you are participating in this study, keep the following in mind:

* This is a study which can give you a good insight into your overall health, based on your responses.
* Find 10 minutes when you are not likely to get disturbed and fill in the questionnaire.
* Fill in the details with information as they are, NOT as you wish them to be.

The Wellness Quotient™ is a number between 1 and 100. A high WQ™ (Wellness Quotient™) indicates a well balanced and healthy lifestyle. Your WQ™ should not in anyway interfere with the recommendation made by a qualified medical professional, which maybe specific to your health condition.

What does your WQ™ mean:

* Above 85: Healthy lifestyle. Adopt preventive practices to continue enjoying a healthy lifestyle.
* Between 70 and 85: Needs attention. A healthy lifestyle can be brought in, provided you make some quick changes with the help of a wellness expert to your current lifestyle. Negligence at this this level can lead to serious health issues.
* Below 70: Needs action. Your health needs some serious attention. It is highly recommended that you create a plan of action and make some changes to your current lifestyle. Work closely with a wellness expert and a qualified medical professional if your WQ score is in this range.
