申請流程 Applying process
1. 填寫本表單  
     Fill in this application form.
2. 收到館員確認信並回覆代檢書目清單  
     Receive confirmation email and send your bibliographic list to the librarian.
3. 收到取件通知
     Receive the pickup notification.
4. 至醫圖i服務台繳費取件  
     Complete the payment and pick up the document at information desk.

收費標準 Service Charges
1. 本校人士：每十篇收費100元，不足十篇以十篇計。
     NTU faculty and students will be charged NTD100 for every 10 articles.
2. 校外人士：每十篇收費200元，不足十篇以十篇計。
     External applicants will be charged NTD200 dollars for every 10 articles.
