YG Sign Up Middle School Gr. 6 - 8
Do to Covid-19 and planning purposes we need to know if your child is coming to youth group. We would like to be able to meet in a group of 10 including leaders and sign up helps us to know if we need to physical distance or not.

Please let us know if your child will be attending youth group on Tuesday for Middle School (Gr. 6 - 8)

Arrival Time: 1pm
Pick Up Time: 2:30pm

Just a few quick notes:
- Bring water bottle
- Mask (on hand incase child develops symptoms while at YG or if we are not able to physical distance)
- Questionnaire to be signed and filled out every time child attends YG at drop off  - Parents need to sign for children 12yrs or younger. Children 13+ can sign their own form.
