Out-of-District Student Enrollment Request Form
The Out-of-District Student Enrollment Request Form must be completed (one per student) for consideration for enrollment in the Evergreen School District for the 2024-2025 school year. This Request Form is for attendance at East Evergreen Elementary School or Evergreen Junior High School. This form is not to be used or applicable for the Evergreen Crossroads Program. Once this completed form is received, you will receive an email notification informing you of the date of the Board of Trustees meeting when your request will be considered; you are invited to attend that board meeting if you would like. After the Board of Trustees has taken action on your request, you will receive an email notification of the decision of the Board and any necessary next steps. Completion of this form does not constitute enrollment in or agreement to enroll in the Evergreen School District. You must complete one form for each student you are requesting to enroll.
