I authorize Big Fish Academy, LLC to electronically debit my bank account according to the terms outlined below. Starting on August 5th 2022(or the day I submit this form if after that date) which pays for current months tuition. Your bank account will be debited the below selected amount(s) on the 5th of each Month for the number of Mondays in each month. Drafts will run on the 4th if the 5th falls on Saturday & on the 6th if the 5th falls on Sunday. I understand that the only prorated tuition months are July, it will bill for 3 weeks tuition and December, it will bill for 2 weeks tuition. I understand that all tuition must be current when the autodraft starts. I acknowledge that electronic debits against my account must comply with United States law.
