Thank you for taking part in our survey about exercise sessions provided by the Berkshire MS Therapy Centre.

We are looking to show the difference exercise makes. Your answers will be grouped collectively and anonymously. We may use your words but we will never do this in a way which you may be identified.

The main use of the survey will be to attract funding to ensure we can continue to provide the service, but your answers may be shared with other health care professionals contributing to the larger conversation about MS treatments.

If there is any question you do not want to answer please move on and any comments you would like to make are more than welcome.

The questionnaire has 28 questions, but on most, you only need to tick a box and the questionnaire should take between 10 to 20 minutes to complete. We are very grateful for your time and effort.  

Once completed, please return to the office by 18 July 2021. If you need help to fill in this questionnaire, please feel free to ask a relative or carer to help. The survey is available online and in paper form.

Everyone who completes the survey by 18 July will go into a prize draw for a £25 John Lewis/Waitrose voucher.
