This form is FOR ALL SOTI SPORTS. It should be used to request a ride for your child to any 4:15p away game or earlier; as well as any practices held off campus. Make sure you are filling out the information properly so we can assign your child a driver (that includes your email). Assistance will only be provided when scheduling rides to games or practices. Once you are notified of your driver, you will need to arrange a ride home if necessary. If your child needs a ride to more than one game, please fill out this form for each game individually.

*Dismissal time for away games scheduled for 4:15p and earlier will be at 2:30p (unless indicated otherwise)

*We do not have carpools for games with start times later than 4:15p
