Thank you for registering your child in Winners Edge Spring Break Camp 2021! We are committed to making your experience with Winners Edge amazing! Please read the terms and conditions below and then sign and date the bottom.

By signing this agreement, I am enrolling my child in a Winners Edge International’s Spring Break Camp, also referred to as “WE” I hereby give permission to WE for my child participate in a variety of outdoor and indoor sports activities, jumping in an inflatable bounce house, transportation in WE vehicles, off site field trips, transportation for field trips, and other physical activities that may not be listed here. I understand that there are certain risks of injury inherent in the practice and play of the physical activities that are offered by WE, as well as in traveling in WE owned vehicles as well as WE contracted vehicles and other related activities incidental to my child’s participation and I am willing to assume these risks for my child and myself. I hereby certify that my child is fully capable of participating in the WE summer program and that he/she is healthy and has no physical or mental disabilities or infirmities that would restrict full participation in these activities, except as included in writing with this waiver. In addition to giving full consent for my child’s participation, I do hereby waive, release, hold harmless, and covenant not to sue Showdown Training Center LLC; Winners Edge International, LLC; Julie Smith Gold Medal Academy; Center for Developing Kids Inc; Mat Surf LLC; Jeanna Plessen; Julie Smith; or any of its owners, officers, staff. coaches, employees, sponsors, supervisors, or representatives for any injury or loss that may be suffered by my child during the participation in the activities included in WE sponsored training courses, camps or afterschool programs, whether the result of negligence or any other cause. By enrolling in the Winners Edge International summer camp, or any of the WE sports classes, I acknowledge and agree to these policies by signing and dating this agreement below.

Memberships: I understand that all memberships are non-refundable but may be transferable with 14-day written notice. I understand that no refunds or credits will be issued if my child misses any days of the WE Summer Camp, except in the case of extended absence due to serious illness, confirmed by a medical doctor. Winners Edge accepts cash and credit card payments. Checks will only be accepted for advance payments.

Payment Options: Weekly payment arrangements may be available on an individual basis, depending on when the child enrolled. Tuition is collected by Winners Edge International seven days prior to each weekly session. If on weekly payment arrangement, I understand that if my tuition is late due to an expired or invalid credit card, or insufficient funds, the WE billing system will automatically charge my account a $15 late fee. I understand that I must pick up my child by 5pm (part-time camp) or 7pm (full-time camp) each day. Unless prior arrangement, at 7:05pm my account will automatically be charged $2 per minute that I am late.

Sick Policy: If my child is sick, he/she will not be allowed to return to WE until 24 hours have passed after my child’s last symptoms. If my child obtains a minor injury, I give permission for Winners Edge staff administer first aid &/or seek medical attention.

Field Trips: On certain camp days, field trip experiences are optional and will cost an additional amount to participate in individual activities. (Parents will be notified in advance of any additional charges for field trips.) Participation in included field trips is mandatory (meaning no child can stay at base camp). My child is required to wear a lime green Winners Edge T-shirt on each field trip.

Photo Release: I understand and agree that my child may be photographed and video recorded for marketing purposes. I also agree that no monetary compensation will be given for the use of these photos or videos and they are the property of Winners Edge International LLC. (Cross out this section and Initial if you DO NOT WANT your child’s photos/videos to be used for promotional purposes.)

Lunch & Snacks: I understand that lunch programs may be purchased from Winners Edge. If lunch program is not purchased, during camps, my child is required to bring a bagged lunch and water bottle per day. Occasionally, daily events will include food/drinks that will be provided by WE. In this case, no extra charges will occur to your account. Winners Edge will provide two healthy snacks per day and offers additional snacks and drinks for $1 each that my child may purchase for cash. I agree that if my child does not bring a water bottle, one will be provided for $1, and that amount will be charged to my account.

Personal Items: Winners Edge is not responsible for the repair or replacement of personal items including but not limited to: gaming devices, toys, clothing, etc. Families are encouraged not to send their children with expensive items to camp.

Sunscreen: Due to Health and Safety regulations, staff members are not permitted to apply sunscreen without permission. Please send your child in sunscreen, and instruct your child about how to reapply throughout the day, and leave a permission slip on file for reapplication.

Pickup: To ensure the safety and security of my child, I understand that any approved adult on my emergency contact form may need to provide a proper state or government issued ID in the event that they have to pick up my child from Winners Edge.
