Please provide confirmation of attendance at any other after school clubs. This will enable the Finches staff to take and collect your children from other extra-curricular clubs that are in operation at the school.

I agree to commit to the sessions requested in this form on a rolling basis

I agree to provide 4 weeks' notice if my child is going to leave or if I need to make alterations to the number of days that they attend

I understand that invoices will be issued termly and full payment must be made by the start of the following term, unless an alternative arrangement has been made in agreement with the club.

I understand that if I do not use any days requested in the booking form, that I am still liable for the fees and that no refund will be given

I understand that if I am late picking up my child that I will be charged a £10 fee, per child, for every 15 minutes from 6pm. This fee will be charged per 15minutes, regardless of when the child is collected
