I realize that lacrosse is a vigorous physical activity that involves body contact, rapid directional change, and running for a long period of time. I understand that participation in lacrosse involves certain inherent risks and that regardless of the precautions taken by the Hastings Lacrosse Club or the participants, some injuries may occur. I am fully aware and appreciate the risks, including the risk of catastrophic injury, paralysis, permanent disability and even death, as well as other damages and losses, associated with participation in a lacrosse event which might result not only from their own actions, inaction or negligence, but the action, inaction or negligence of others, the rules of play, or the condition of the premises or of any equipment used. Further, I agree that there may be other risks not known to us or not reasonably foreseeable at this time.I agree on behalf of myself, my heirs, and personal representatives, that Hastings Lacrosse Club, the host organization, and sponsors of any Hastings Lacrosse Club event, along with coaches, officials, referees, umpires, volunteers, employees, agents, officers and directors of these organizations, shall not be liable for any injury, loss of life or other loss or damage occurring as a result of my participation in the event. I assume all foregoing risk and accept personal responsibility for damages following such injury, permanent disability or death.

My child/athlete has my permission to be photographed while participating in Hastings Lacrosse. Photos may be published in Hastings newspaper, website and social media.
