Academy Placement Survey SY2021
Dear Academy Families,

At Academy Avenue, we value your input regarding your child’s academic and learning needs.  In an effort to be thorough and to give your child the best possible learning conditions, we recognize that parents/guardians have a special knowledge about their children that we would like to include in the placement process.  You may have information relative about your child’s personality, social, emotional, health concerns, and historical background that will aid us in her/his placement.  Given the circumstances with COVID-19 and schools being physically closed, we will be collecting this information electronically this year. If you have more than one child, you can submit this form more than one time.

Please take a few moments and complete this gForm by Wednesday, May 13th. Your child’s current teacher, the office team, and next year’s teacher will use the information you provide to help us with the placement process.  It should not contain a request for a specific teacher.  Final placement decisions will be made by the principal for the 2020-2021 school year.  Placements will be shared out to our families in August (pending any changes given the current situation).  Thank you for your help in this process as we want all of our students to thrive.
