Please complete this form if your student is currently a remote learner and intends to return to the physical building for the fourth quarter. You will need to submit a form for each student who will be returning. The fourth quarter begins March 15, 2021.

This form is NOT for:
1. Students who are not going to return at the beginning of the second quarter.
2. Traditional students who are currently coming to school every day.

The deadline to submit this request is Friday, February 26, 2021.

*Please Note - Students who will be transitioning from remote to traditional learning will stop by the front office on March 15, 2021 to receive their schedules!
