Moana Summer Camp Registration
Use this form to register for Moana.  Once you have completed this form we will email the tuition payment links if you are a new student.  All students who enroll are accepted until the program is full.  Parts are assigned by Audition.  We are only having one cast and do anticipate spots filling quickly as they have the past few years.

Camp Meets Monday - Friday, July 8th - 12th & July 15 - 19th
Monday -  Thursday July 8th - 11th from 9:00 am to 2:00 pm Monday -  Thursday July 8th - 11th 
Friday, July 12th 9:00 am to 11:30am (Musicpalooza performance at 6:00 pm Friday, 7/12)
Monday - Thursday July 15th - 18th 9:00 am to 2:00 pm
Friday, July 19th  we will have camp at Lincoln-way West from 9:30 am until 1:30 pm
The show is that evening at Lincoln-Way West

On July 12th there is a community sneak peak at All About Musicpalooza, part of the Mokena Lions Club Firecracker Weekend.  Held under the giant tent on Front Street, food drinks are also available.  Admission is a donation for the Manteno Veteran's home or $5 cash per person attending.  A list of most needed items will be provided at the start of camp.

The show will be at Lincoln-way West High School on Friday, July 19th at 7:00 pm
Tickets are $12 - $16 each.

Tuition is $450 for new campers and includes a camp tee shirt.  Tuition for current AAMCT members (cast of Little Mermaid, Madagascar or Aladdin) is $430.

VOLUNTEER OPPORTUNITIES.  In order to have a successful production we require the help of parents and/or additional staff for the shows.  If you are unable to volunteer please include the $25 volunteer buyout with your tuition.   Please select a minimum of THREE opportunities below.  YOU MUST INCLUDE EITHER MOVE IN OR STRIKE AS ONE OF YOUR CHOICES. *

RAFFLE BASKETS  We will be having a raffle before the show and would love to have baskets that coordinate with the show.  Please let us know if you would like to donate a basket or items for a basket.  We are also planning on another lottery board/basket if you would like to donate lottery tickets.     *

PUBLICITY RELEASE.  Portions of  performances and rehearsals may be photographed and/or videotaped for promotional purposes including but not limited to web site, brochure, newspaper ads/features, social media,  or promotional & archival videos. Please check the box indicating All About Music has permission to use your child’s image/name. *

PERMISSION SLIP AND HOLD HARMLESS CONTRACT My son/daughter who's name I have entered above has my permission to participate in the production of  “Disney's moana Jr” produced by All About Music, Inc. (herein referred to as the Company). As Child’s Parent or Guardian, I release the Company from any and all liability, damages, or claims whatsoever for any   injury or harm that may occur to my Child while participating in any rehearsal, activity or performance provided by the Company. I agree that I will make no claim or demand against the Company or any organization or facility used for rehearsal or production purposes. If an injury or accident occurs during any rehearsal, activity or performance   provided by the Company, I will look to my own resources, insurance or assets to pay all medical bills, damages or losses whatsoever if any injury or incident occurs. The term Company includes all employees, subcontractors, volunteers or other staff of the Company participating in “Disney's Moana Jr.”  I give my permission to release my child to my emergency contact listed above. I also agree to pick my child up no later than 10 minutes after the designated end of rehearsal or I agree to pay standard day care fees for each 15 minutes I am late. I also agree that if my child exhibits disruptive or disrespectful behavior, that  cannot be corrected after being addressed with myself and my child, my child will be removed from the production and no refunds will be given.  My typed name will serve as proof of my agreement to the hold harmless contract stated above and my legal signature *
Once you have completed your registration form you will be emailed the tuition payment links.  Your enrollment is not complete until you have paid the tuition fee.     A full refund is available minus a $25 processing fee until May 1st. A half refund is available until June 1st.  No refunds are available after June 1st.   *
