Only one person from your group needs to fill out the form! Please choose a date that works best for you and your group, if you're working with an artist and/or co-author! There are no limits on how many people can sign up for a chosen date, so please just choose the date that will work best for you and your group. Daily reveals are set to be at about 5PM MST.

You will receive a copy of this form to the email address that you leave below within 24 hours, if you do not, please reach out to us at dcubang@gmail.com. The form will be sent to the email address you put in below, if you typo your email address, you will not receive a copy of this form. Please make sure to input the email address you used to sign up, otherwise I cannot mark your reveal date claim!
