Instructions
  • For families, kindly fill out separate forms per family/group member. However, for those filing a Report of Marriage, one spouse may fill out this request form for both spouses. Please avoid submission of duplicate requests per person. 
  • Clients are also requested to ensure that all information shared through this ARF are accurate and are consistent with the relevant documentary requirements needed for their requested transactions.
  • The Embassy will reach out to clients for their respective appointment schedules in due course. 
  • To ensure smoother coordination on your requests, kindly regularly monitor your mobile phones, email inboxes as well as Spam/Junk folders for the Embassy's notice regarding your request/s.
  • Due to limited slots, not all registrants may be accommodated. 
  • Venue and time of the Consular Outreach Mission will be conveyed in due course.
