This order form is to complete a request for a pose to be applied to the graphic background.
Steps:
1. Go to the gallery and pick a pose
2. Hover over the image in gallery to get the image file name. The file name will be the name of the player and a number (ie NDampson 01)
3. Come back to this form and fill out the information
4. Once the image is created, the final product will be uploaded into the gallery for purchase or print order
- Allow two weeks from submission of form for images to be completed and uploaded into the gallery
Digital Downloads are NOT to be ordered with this form. Complete those purchases in the gallery.
Clients can submit more than one form as needed to request a graphic background or order prints.
