To request a private lesson with Jackie please fill in the spaces on this form with your name, email address and the times you are available for. Private lessons cost £30 for 45-minute slots. If you attend with your partner, you can split the cost between you. Please pay Jackie directly in cash when you come for the lesson.

Jackie will be available 10am to 5:45 pm.

The submission period for this form is Friday 15th March at 10am to Saturday 16th March at 12pm. Submissions before or after this time period will not count.

Privates are allocated on a first come first served basis, so please fill out the form early in the submission window, particularly if you need the lesson as preparation for a competition or demo. If you aren’t able to get a private lesson, you will be put on the priority list for the next time Jackie is in.

An email will be sent out to you confirming whether you have a private lesson, the time and the room by at latest Sunday 17th March.  

If you need to cancel your lesson, please let me know asap so that I can find someone to fill the slot. If you cancel before Monday at 12 pm, you and your partner will still be expected to cover the cost of the lesson if we cannot find someone to fill the slot. We do understand that things can come up, however this does lead to a loss of earnings for our coach.

We will use and store your data in accordance with the Data Protection Act 2018 and Guild of Students’ Privacy Notice which you can find at guildofstudents.com/privacy. You can request for us to delete your data at any time by emailing ballroom@guild.bham.ac.uk, or by talking directly to the committee
