I hereby authorize the coaches of the Curt Bly Baseball Camp to act for me according to their best judgment in any emergency requiring medical attention.  The coaches at the Curt Bly Baseball Camp are acting as independent agents.  I hereby waive and release Curt Bly, the camp coaching staff, Mason City Schools Board of Education from any and all liability for injuries incurred while at camp.
