By submitting this completed form you agree to the following items:

* I understand that I am offering my services as a volunteer and I am not receiving any monetary compensation for my time.

* As a representative of The Oregon Center of Creative Learning, including The Children's Museum of Southern Oregon and The Ivy School, I will conduct myself in an open, friendly, and professional manner and I will present myself in a way that is inviting and appropriate for children.

* I agree to abide by the policies and procedures set forth by The Oregon Center for Creative Learning. I understand that I may not be unsupervised with children for any reason. I also may not hold or lift a child while volunteering. I am not responsible for discipline and will leave this to the professional educators.

* As a volunteer for The Oregon Center for Creative Learning, a non-profit organization, I accept sole responsibility for any injury that I may incur during the time I am volunteering for the organization; at a special event; or on property owned or operated by The Oregon Center for Creative Learning. By signing this waiver, I further release The Oregon Center for Creative Learning and their employees and/or agents from any claims or causes of action which may arise from any accident or injury caused by any reason. I understand that The Oregon Center for Creative Learning is not responsible for loss or damage to my personal property.

* I authorize The Oregon Center for Creative Learning to photograph or videotape me for use in any and all media (including photos on websites, Instagram and in publications) without compensation. This media is the property of The Oregon Center for Creative Learning and I release The Oregon Center for Creative Learning, its successors, assigns and licensees from any liability.

* This waiver is valid for one year from the date of submission.
