Parent/Guardian & Student must complete this form together.  Have your credit card information ready as you will be prompted to enter payment information at the bottom of the consent form.  Please read each section of the form carefully and fill in all required fields. EVERYONE must fill out this permission form.

There is a section for parents and a section for students, however the email address to fill out this form MUST be the one of a parent, NOT a student.  We have included important information for the trip below.

Time: Students will arrive at school at 8:30 am.  Students are expected to arrive back at school around 4 pm and must arrange for a ride home from there.

Cost: $ 40.00

The cost of the trip is $40.00 per student, which includes:

                   - Roundtrip transportation via bus and subway

                   - Admission to American Princess for Adventure Cruise

                   - Free school lunch ( sandwich, snack and fruit)       

Bring :      1) $$- Snack concession and a full beverage bar on board; sandwiches are permitted to be brought                            on board, however beverages and coolers are not.
