We are glad you are interested in attending the indoor worship service at Mountain View scheduled for May 23, 2021 at 10:00 am.

We ask you to please answer the questions below. If you are not feeling well or have not felt well recently before the service, please stay home. A recording of the service will be posted online shortly after and you'll will be able to view it remotely.

This service is subject to change or cancellation depending on current conditions and guidance.

We also ask you to make the following commitment: If I attend indoor worship or small group event at Mountain View, and subsequently am diagnosed with or presumed positive with COVID-19, I will report this to the office at Mountain View (office@mtview.org or 303-494-5025). I understand that my identity/health status will be kept confidential, but that my name/phone number and the list of those also attending with me may be shared with Boulder County Public Health for contact tracing purposes.
