If you need any help with completing your child's enrolment, our friendly staff are available to assist.  

Once this form is completed, you will need to provide a copy of the following as soon as possible to the school office:

- Your child's birth certificate
- Your child's immunisation certificate (from Medicare or an Australian medical doctor)

If your child was born overseas, you will also need to provide a copy of your child's passport and visa.
