❀ Thank you for your interest in my work and wanting to make a tattoo appointment with me! Please don’t rush the questions, I will not go with “first come first serve”. Your personal info will only be seen by me, I will not use it for any other purpose than to help you with your tattoo.
❀ I can't reply to forms that are incomplete. Please be specific with your answers and try to give the best description you can of what you have in mind for your tattoo :)

❀ The form can be filled out in English or Dutch!

A few things to consider before filling out this form:

❀ Due to the amount of interest I receive for my work (thank you!) I will only accept tattoo projects I feel like they fit best within my personal style of tattooing! I always try to fit in as many people as possible, but I can't guarantee it. Preferred projects include floral/botanical work, as well as anything anime/geeky/gaming or japanese related. I have limited spots available for this booking period!

❀ I ask a deposit for your tattoo appointment, please consider this as well. Deposits are NON-REFUNDABLE.

Due to the current COVID situation I am not taking on lots of consultations in the shop at the moment. I will provide all the info you need for the appointment through email. For big projects like sleeves we can always arrange something. Please let me know if you have a big project you want to start so we can discuss the options!

Try to describe your tattoo idea as detailed and specific as you can. Please be more specific than "I want a flower tattoo". Include all the elements you'd like to see in your tattoo.
Please keep in mind; I don't copy tattoos or designs from the internet. Once I email you back I will ask to send me some reference photo's of tattoos you like as inspiration. I will also ask to send a picture of your own arm / leg / etc. - this helps me a lot in the design process
What will be the size of the tattoo? (in centimeters) *
Please get a ruler and measure accordingly! The price will be based on the size aswell. Measure the length and width in centimeters. We will make a perfect fit on the day of the tattoo appointment but I need a good estimate beforehand. Please note that the price is also based on size, so that's why it's important to measure beforehand :)

Please check your preferred days down here! The more flexible you are with planning the better! :) Please note I don't work evenings. My Saturdays fill up very fast so make sure you are available on a weekday as well!

Once you've applied this form I will email you back regarding the booking proces. I do my planning work after work hours (by myself) so it can take a while. Patience is key :) The waiting time for a reply can be up to 3 or 4 weeks!
