RELEASE: I/We do for myself, each player and her parents, release, discharge, and agree to indemnify RBC and its coaches, board members and agents (collectively referred to herein as "RBC") and the sponsors/site of the practices/tournaments, and the owners of facilities in which the player practices or play, from all claims and possible liability for injury, loss, damages including past and future bodily/mental/emotional injury and other damages or loss suffered by the player and by the undersigned and by others, and which now exist or may hereinafter be discovered or arise from any claim, including claims that are alleged to be caused, in whole or in part, by RBC's neglect, failure, fault, omission, acts, statements or otherwise. RBC is released from all responsibility or liability for all loss, damage, expense, injury or damage incurred or suffered by the undersigned and/or by the player for any reason, including the negligence of RBC, its coaches, volunteers or board members. This is a continuing indemnity and release of all existing claims and all future claims that may hereinafter arise, whether now known or not known. This agreement is binding on us, the player and each of our respective heirs, executors and administrators.

EMERGENCY CARE: I/We hereby consent to and authorize RBC, its coaches, volunteers, any licensed physician, EMT or other qualified hospital personnel to render medical treatment to the player in an emergency. I also authorize them to transport my daughter to a medical facility to which, in their judgment, is necessary in the event of illness or injury. All medical and related treatment costs are my sole responsibility, and not that of RBC, or its coaches or board members. I will hold RBC harmless therefrom.

PERMISSION: The undersigned, as the parent/guardian of the above named player(s) consents to have the player(s) participate in all RBC programs, practices, games, trips, scrimmage and other activities, which include practice and scrimmages against players/teams of different age/sex, people of larger size and older ages than the player, and also to travel out of town for scrimmages and games, and to participate with/against other players on one or more RBC teams. Each player's physical/mental condition changes such that the player should not participate, I/we will promptly notify RBC and the team's head coach. I/We understand that activities and events in the program and competition involve various risks of injury or loss, and I/we assume all of the risks of injury. I/we give RBC permission to use my child’s photograph publicly to promote the program. I /we understand that the images may be used in print publications, online publications, websites, and social media. I/we also understand that no royalty, fee or other compensation shall become payable to me by reason of such use.

WAIVER: In consideration of being allowed to participate in any way in the RBC athletics/sports program, related events and activities, the undersigned acknowledges, appreciates, and agrees that: 1. The risk of injury and/or illness from the activities involved in the program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce the risk, the risk of serious injury does exist; 2. The risk to have contact with individuals, who have been exposed to and/or have been diagnosed with one or more communicable diseases, including but not limited to COVID-19 or other medical conditions, diseases, or maladies does exist, and it is impossible to eliminate the risk that I could be exposed to and/or become infected through contact with or close proximity with an individual with a communicable disease; 3. I/WE KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume all full responsibility for my participation; 4. I/we willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I/we observe any unusual significant hazard during my presence or participation, I/we will remove myself from participation and bring such to the attention of the nearest official immediately; and 5. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS RBC, their officers, officials, agents and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of the premises used to conduct the event ("Releasees"), WITH RESPECT TO ANY AND ALL INJURY, ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE. 6. I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, BEFORE ACKNOWLEDGING THE CHECKBOX BELOW, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY AGREEING TO IT ON MY OWN BEHALF OR ON BEHALF OF THE YOUTH PARTICIPANT ASSOCIATED WITH THIS GUARDIAN ACCOUNT, AND I SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

ACKNOWLEDGMENT BY PARENTS AND/OR LEGAL GUARDIANS OF YOUTH PARTICIPANTS: By acknowledging and agreeing to the checkbox below, I agree to and verify the following: 1) I am the parent or legal guardian for the youth participant associated with this guardian account, 2) that the date of birth of the youth participant associated with this guardian account is correct, 3) that as parent/legal guardian with legal responsibility for this youth participant, I consent and agree to assume the risks of his/her participation in these programs; and 4) that I specifically agree to his/her release as provided herein of all the Releasees, and, for myself, my heirs, assigns and next of kin, I release and agree to indemnify the Releasees from any and all liabilities incident to this youth participant's involvement or participation in these programs as provided above EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE.
