How did you hear about this scholarship?
The scholarship award for which I am applying may contain funds donated to local chapters, WAA, UW-Madison or the UW Foundation. At times, local chapters may need information regarding applications that may include grade point average. I agree to give my consent to release this information for the duration of my attendance at UW-Madison.
