Dear Families,

Welcome to a new school year at J.T. Henley Middle School! Throughout the school year, many community groups, along with Henley's PATSO, support families needing assistance with food, toys, and other necessities. We cannot guarantee that you will receive assistance, as this form is distributed to all families, however, we would like to know if your family has a need so we may begin to organize assistance.

If your family could benefit from these programs, please complete this form. Please complete this form even if you completed a family assistance form at another school. Because this information may be shared with outside organizations, please sign below to give us your permission to allow these organizations to fill your requests.
