LevelUp! Roundtable

Thank you for your interest in the Roundtable! We're excited to see how this community comes together and supports one another in the LevelUp! process.

This is the application for membership in the next 6 month cohort (date TBD) This form provides additional information about the program including the investment and commitments you're making in your success. The standing meeting time is currently planned for the first Tuesday of every month at 1pm Eastern time.

If you're accepted, we'll invoice for the investment of $4,450.00 (US, due on receipt) which includes:

- Program setup (including an invitation to the online community)

- Everything DiSC Workplace Profile survey and personalized report

- Initial business coaching session

- Ad hoc Email & SMS coaching support

It also secures your position in the cohort for 6 months.

If you're unable to agree to all the items, it might be that this type of group isn't a good fit for you yet. If you have a question or concern, please contact us so we can explain how it works.

I am ready and am committed to doing the work to LevelUp. I will be fully present in the group, adding at least as much as I'm getting.

I will hold all information from other cohort members in strictest confidence. I will not disclose information shared through the RoundTable without explicit permission from the person who discloses it. I will presume confidentiality for everything. This is the "Four Virtual Walls" rule.

I understand this is a peer support forum and I'm solely responsible for all actions and decisions I make based on the feedback of others.

I will model integrity, kindness, and generosity within this community and will do my part to establish a culture of empathy and respect. I recognize that failure to do so may result in forfeiture of my place in the community.

I agree that being accepted into the cohort is also contingent on payment of the program fees.

I agree that if I must be administratively removed from the Roundtable for violating these commitments, I will not be eligible for a refund.
