1. 所有使用者／觀眾在進入場館前，必須佩戴自備的口罩，及須符合「疫苗通行證」實施的相關要求，並利用手機掃描「安心出行」二維碼，豁免人士除外。
All venue users/audiences, save for those exempted, are required to put on their own masks, comply with the requirement of the Vaccine Pass and scan the "LeaveHomeSafe" QR code before entering the venue.

2. 此表格用作登記預留本節目門票。門票數量有限，每人限取 2 張。先到先得，額滿即止。每個電郵地址只限登記一次。
This form serves for the registration of admission ticket of the captioned programme.  Free tickets are distributed on a first-come-first-served basis, each patron may get a maximum of 2 tickets.  Each eligible email account can only be registered once.

3.成功登記人士將會收到確認電郵。如有疑問，請於辦公時間內致電2591 1340 或電郵 cpo_enquiry@lcsd.gov.hk 查詢。
Tickets will be issued via e-mail upon successful registration.  For enquiries, please contact the Office at 2591 1340 during office hours or email to cpo_enquiry@lcsd.gov.hk.

1. 你所提供的資料將會用作「康樂及文化事務署社區節目辦事處」處理活動報名事宜、日後聯絡及意見調查的用途。The information collected will be used by Community Programmes Office of the Leisure and Cultural Services Department to confirm your registration, to contact you for our performances and activities, enquiries of feedback,  and/or statistical or administrative purposes.

2. 除個人資料(私隱)條例所訂明的豁免外，根據同一條例第18及22條及附表1第6項原則，你有權要求查閱及更改本表格內所載列的個人資料。Subject to exemptions under the Personal Data (Privacy) Ordinance, and in accordance with Section 18 and 22 and Principle 6 of Schedule 1 to the Ordinance, you have a right of access to and correction of the personal data provided in this form.

3. 查閱及更改個人資料的要求應以書面形式向「社區節目辦事處」提出，詳細聯絡資料如下 Requests for personal data access and correction should be addressed to the Community Programmes Office：
