BAWSI Terms [Terminos de BAWSI]

I give permission for my child to attend and participate in BAWSI Girls.   [Doy permiso a mi hija de asistir y participar en el programa de BAWSI Girls]

I understand and acknowledge that running poses risks to my child, including the risk of strains, sprains, falls, broken bones and serious injury or death.      [Entiendo y reconozco que correr plantea riesgos para mi niña, incluyendo el riesgo de torceduras, esguinces, caídas, fracturas y lesiones graves o la muerte]

I attest that my child is physically and medically able to participate in BAWSI Girls.   [Doy fe de que mi hija esta físicamente y medicamente capas de participar en BAWSI Girls]

I understand BAWSI is NOT responsible for my child after the program is over.   [Entiendo que BAWSI no es responsable de mi hija después de que el programa haya terminado.]

I understand that all employees of the Bay Area Women’s Sports Initiative are mandated reporters of child abuse. [Entiendo que todas las empleadas de BAWSI están obligadas a reportar cualquier tipo de abuso infantil.]

I, as parent or guardian of the minor identified above, do hereby, for my child, myself, my heirs, executors and administrators, release and forever discharge and hold harmless BAWSI and all officers, directors, employees, agents and volunteers of the organization, [INSERT] School District, and the State of California, acting officially or otherwise, from any and all claims, demands, actions or causes of action which in any way arise from the minor’s participation and/or my participation in the BAWSI Girls activities.
