The following request form is for the 2021-2022 school year. The coordinator of the program you selected will contact all parents/guardians after review of the request form to provide further information.

1. All parents/guardians must enroll ALL of their children in one account even if the students attend different schools in the district through our Online Registration (OLR) system. For more information on OLR, please visit:

If you are requesting a program at a school other than your school of residence (according to your home address) you will also need to apply for an Intradistrict Transfer during our Open Enrollment period (April 5 - May 7, 2021).
