Family Survey for AYES - Fall 2020
Dear parents and guardians,
In anticipation of a possible transition to on-campus instruction, we are asking that you complete the following survey by Wednesday, Oct. 14. We also request that if you have more than one student at Anna Yates Elementary School, you fill out one survey per student. Thank you for being a great partner in educating your children!

Very important
Somewhat important
Not an important factor
A steady decline in infection rates in Emeryville in the prior 14 days
An effective coronavirus vaccine is available
Testing is available for schools to randomly monitor for asymptomatic virus
Staff and students wear face coverings
Scheduling is adjusted to provide outdoor learning as much as possible
Facility deep cleaning systematic and ongoing
Daily temperature checks for staff and students
Additional handwashing and/or hand sanitizing stations have been installed
Classroom seating is limited to maintain 6 foot distances
No more than 12 children in a classroom
Siblings could attend with the same schedule
Student groups are kept in the same pods
All transitions, including the school start time, are staggered to accommodate the pods
If social distancing and cleaning requirements make it difficult to have rotating cohorts on campus, another option is to have a smaller percentage of students on campus part-time, with most students in distance learning. Priority for space on campus could go to children with greater needs, due to their parents’ employment or other student or family circumstances, and some students might attend on a space-available basis. Please indicate your response to this idea: *
Once the district moves to the Hybrid Model of instruction, I will need child care for the time when my child is not on campus. *
When the district will transition to a Hybrid Model, are you planning to send your child back to school or will you keep your child in distance learning?
What else would you like the district to know as we prepare to move to the Hybrid Model of instruction, ACPHD-permitting?
