While we strongly believe that in-person learning will best serve the great majority of our students, we understand that in limited cases, certain students may face specific health risks with in-person learning due to COVID-19. Those risks include:
- Severe allergic reaction (e.g., anaphylaxis) after a previous dose or to a component of the COVID-19 vaccine
- Immediate allergic reaction of any severity to a previous dose or known (diagnosed) allergy to a component of the vaccine
- Health condition which places the student at greater risk of severe illness from COVID-19 as outlined by the Centers for Disease Control (CDC): Medical complexity; Genetic, neurologic, metabolic conditions; Congenital heart disease; Obesity; Diabetes; Asthma or chronic lung disease; Sickle cell disease; Immunosuppression

If applying for more than one child, complete a separate application for each child.
Applications are reviewed by ICSD medical personnel.
